As per a latest report by government only 16% GSTR-1 and GSTR-3B returns matched. What do you think is the major reason for not matching of Returns on such a large scale?

Possibly due to software mismatch and technical errors.

The major reason is that in case of transportation services outward supplies covered under reverse charge are not shown in GSTR3B where as the same is declared GSTR1. Another reason is that many times it happens that there are certain mismatch as per GSTR 1 report in tally as compared to GSTR3B , where ther is no mismatch for same period , hence the data gets changed when we rectify the mismatch in GSTR1 , but the same can be avoided if before filing GSTR3B we counter check for mismatch arising in GSTR1.

Difference of Debit and Credit Notes. In case of traders of Coal the TCS will be added to the taxable value and hence the invoice value will not be taxable value plus gst.

    1) The time period to compile the date is very less. It is possible to compile data only of dealers who are doing real time accounting. People working in dual mode that is offline and online are unable to cope with the time limit.
